The privacy and security of your information is important to Gift of Serenity Massage (GSM). This describes what personal information we collect, and what we do with this information. There are two categories in this policy: personal information for GSM and how the website interacts with your computer.
Personal Information for GSM
Primary Purpose of Collecting Personal Information:
GSM collects your phone number and email in order to confirm appointments made with email or online booking on this website.
Once in the studio, a health intake form is completed (e.g.,name, address, contact info, health history, health conditions, and emergency contact.) GSM collects and uses this personal data in order to provide treatment and to provide the therapist with a baseline of health and lifestyle information (e.g. type of work, sports, exercise). This is necessary in order to design an initial treatment plan and an ongoing plan as your needs change.
Secondary Purpose of Collecting Personal Information:
GSM may share personal information about a client with other health care providers (e.g., medical doctors, chiropractors, physical therapists) with whom the client has a relationship. Generally, these providers have privacy policies for the client’s protection.
How the website relates to your computer
We do not collect any personally identifiable information when browsing our site. We may, however, collect information such as time of day, browser type and IP address. This information is used to make improvements on our site and enhance the user experience.
A third party business is used for online booking and gift certificate sales, their privacy policies apply. See MassageBook for their policies.
Protecting Personal Information
Paper information is in a restricted area and is securely destroyed after it has been stored electronically.
Electronic hardware is in a restricted area and passwords are used on computers.
GSM keeps client files to ensure we can answer any question the client may have about the services provided and for our own accountability to external regulatory bodies. Client files are kept for 7 years after the last client appointment.
GSM destroys paper files by shredding. We destroy electronic information by deleting it and, we physically destroy the storage drive when we discard the hardware.
