Cancellation and General Policies
Cancellations will be accepted up until the time of the session, otherwise the appointment will be considered a “no-show”. Client will be responsible for full payment of any no-show situations. If the therapist is a no-show (misses the appointment without informing the client), there will be no charge to the client.
Payment is expected at time of service. Payment forms accepted include cash, checks, Visa, Mastercard (debit or credit).
As long as you are on time, you will receive the full massage time (e.g., 60 minutes “on the table”). Please allow that time in your schedule as the total session will be 5-10 minutes counting dressing and undressing and payment. If you are on a tight schedule, please make that known at the beginning of the session.
I understand that the massage/bodywork I receive is provided for the relaxation and relief of muscular tension.If I experience pain or discomfort at any time during the session, I will immediately inform the practitioner so that the pressure and/or strokes may be adjusted to my level of comfort. I further understand that massage/bodywork should not be constituted as a substitute for medical examination, diagnosis, or treatment and that I should see a physician or other qualified medical specialist for any mental or physical ailment of which I am not aware. I understand that massage/bodywork practitioners are not qualified to perform spinal or skeletal adjustments, diagnose, prescribe, or treat any physical or mental illnesses and that nothing said in the course of the session is given should be construed as such. Because massage/bodywork should not be performed under certain medical conditions, I affirm that I have stated ALL my known medical conditions and that I have answered all questions honestly. I agree to keep the practitioner updated as to any changes in my medical profile and understand that there shall be no liability on the practitioner’s part should I fail to do so.
Consent to treat a minor
By your signature on the intake form, you authorize the therapist to administer massage or bodywork techniques to your child or dependent as the therapist deems necessary. You as the adult responsible must remain in the session room if the client is 17 years old or younger for the duration of the massage.
GSM requires draping (being covered with a sheet/blanket). Any requests for no draping will be denied. Any requests or comments of a sexual nature will result in immediate cessation and you will be asked to leave.
Policies may change as needed. Clients will be informed of any change in policies as they come to their sessions.
